T — Weekly Change Log
Fifteen minutes on a Friday. The single most useful habit in this toolkit — it’s where your retrospective, your board paper and your credibility all come from.

Why bother
It’s where the retro comes from. At day 90 nobody remembers week 3. The log does — and honest contemporaneous notes beat reconstructed memory every time.
It catches drift early. Three weeks of “blocked on the same decision” is visible in a way that three separate conversations aren’t.
“What I was wrong about” builds the right habit. It keeps you diagnosing rather than defending, and it’s the discipline that separates people who get better at this from people who accumulate experience without learning.
It’s your evidence. When someone says “we never raised that”, the log has the date.
